I'm not thinking a band like GWAR where everyone has a stage name - and I don't mean legal or unofficial name changes like David Jones/Bowie or Gene Simmons/Chaim Witz, either. And not nicknames that the member had before the band, either (I know - lots of rules).
I'm thinking more like Buck Dharma, Fee Waybill, and Perry Farrell - where the name is kind of band-related alter ego that stuck over time (a subtle distinction, I know).
Not sure if this fits your criteria, but I love love love the drummer's name from Steel Panther "Stix Zadinia"

Flea's a good one. Sting, too. But there was something in the late punk/new wave era where they really designed an alter ego for themselves, and seemingly just for their role in the band, that I liked. Siouxie Sioux is another one.
